Syllabus: Prelims Unit I - General Science | Mains Paper III, Unit 2 - Science and Technology in India's Development

What Was Approved

  • The Drug Controller General of India (DCGI) granted market authorisation to Takeda Biopharmaceuticals India's QDENGA (TAK-003), making it India's first approved dengue vaccine, cleared for the prevention of dengue in individuals aged 4 to 60 years.
  • QDENGA is a live-attenuated tetravalent vaccine designed to protect against all four dengue virus serotypes (DENV-1 to DENV-4), administered as a two-dose regimen, three months apart.
  • It can be given irrespective of whether a person has had a previous dengue infection, and does not require pre-vaccination screening.

Why Now: India's Dengue Burden

  • Reported dengue cases in India have risen nearly 11-fold over the past two decades.
  • India accounts for nearly one-third of the global dengue burden, though modelling studies suggest actual infections are substantially higher than what gets reported.

The Clinical Evidence

  • The approval draws on Takeda's global clinical development programme comprising 19 Phase I, II and III clinical trials involving over 28,000 participants across dengue-endemic and non-endemic regions.
  • The pivotal Phase III trial alone enrolled more than 20,000 participants across eight dengue-endemic countries.
  • The vaccine showed 80.2% efficacy against virologically confirmed dengue 12 months after the second dose, and 90.4% efficacy against dengue-related hospitalisation at 18 months.
  • India's own approval was supported by a Phase III trial among participants aged 4 to 60, which found the vaccine safe, well tolerated and immunogenic in children, adolescents and adults.
  • Commonly reported side effects were mild to moderate: pain or redness at the injection site, headache, muscle ache and mild fever - typically temporary and self-resolving.

Update, July 25: The Serotype Science and the Pricing Question

  • Dengue has four antigenically distinct serotypes (DENV-1 to DENV-4). A second infection by a different serotype can produce more severe disease through antibody-dependent enhancement (ADE), so an effective vaccine must induce strong immunity against all four at once.
  • Qdenga's efficacy is highest against DENV-2, then DENV-1, with uncertainty remaining for DENV-3 and DENV-4 in people without prior exposure. This matters for India because DENV-2 is more common but DENV-3 prevalence is rising. The TIDES trial raised concern of "negative efficacy" against DENV-3-related hospitalisation among seronegative children.
  • Unlike Dengvaxia, the first licensed dengue vaccine, Qdenga can be given without screening for prior dengue infection. Dengvaxia was nearly derailed by the 2017 Philippines controversy, which prompted greater regulatory caution worldwide.
  • The schedule is two doses three months apart, which creates a follow-up challenge among migrant workers. Takeda follows a tiered pricing strategy, and the government has been urged to negotiate a lower price for India.

Update, July 27: A New Study Urges Screening Before Vaccination

  • A paper published in Nature Medicine on 25 July 2026 by Imperial College London researchers modelled Qdenga's population health impact and concluded that seronegative recipients, especially children, should be screened for prior dengue exposure before vaccination - a stricter recommendation than the approval terms, which do not mandate screening.
  • At a conservative estimate of 40 million children in India, with under 30 percent seropositive, the number of seronegative children facing this heightened risk could be very large.
  • The underlying Phase III trial (about 20,000 children aged 4 to 16 across 8 countries) showed overall efficacy of about 80 percent (82% in seropositive children, 75% in seronegative children); but stratified by serotype, seronegative children given Qdenga showed more susceptibility to DENV-3 infection than unvaccinated seronegative children - a subsequent natural DENV-3 infection carries about 25% higher likelihood of illness via antibody-dependent enhancement (ADE). DENV-4 efficacy remains essentially unknown due to too few trial cases.

Global Rollout

  • QDENGA has been approved in 43 countries across Asia, Latin America and Europe, with more than 32 million doses distributed through public and private immunisation programmes.
  • The vaccine holds World Health Organization Prequalification status. Medical advisories caution that it may not be suitable for people with weakened immune systems.

Update, August 28: Tamil Nadu’s Case Load and Its Surveillance Network

  • Replying in the Assembly, the Minister for Health said that from January 1 to August 26, 12,856 dengue cases were confirmed in Tamil Nadu, with seven deaths.
  • Dengue spreads only through mosquitoes that bite during the day, which is why the standing public advice is to prevent water from stagnating in and around homes rather than to rely on night-time protection alone.
  • Fever surveillance runs daily at 5,805 government and private hospitals, tracking the number of fever patients, dengue and swine flu patients, clusters, deaths and mosquito breeding sites, with both larval control and adult mosquito control under way.

Update, September 9: A Cluster Randomised Trial at Kadayanallur

  • Tamil Nadu will run its first scientific field trial of dengue prevention at Kadayanallur in Tenkasi district, the Health Minister announced in the Assembly.
  • In 2025 the State recorded 25,580 dengue cases and 12 deaths. So far in 2026 there are 14,581 cases and 9 deaths.
  • A cluster randomised controlled trial assigns whole groups of households, rather than single individuals, to the measure being tested. That is what allows it to measure an effect on transmission across a community instead of protection for one person.
  • A repellent applied on the body is already known to prevent a bite; there is no field evidence that community-wide use lowers the spread of the disease, and that is the gap the study is designed to fill.
  • Three trials will run: to identify the better repellent; to measure the combined effect of a bed net with a repellent applied on the body; and to assess extra protection at the household level. Each has 48 clusters of 20 households, so about 2,880 households take part, at a cost of about Rs. 43 lakh.
  • Beyond effect, the trial measures continued use, public acceptance, feasibility and cost, because a measure that works but is not used changes nothing.

Update, September 22: Two Official Counts of the Same Epidemic, and the Case Definition Between Them

  • Two official series of dengue figures for Tamil Nadu disagree for the same years. The State health department records 56,863 cases for 2023, 46,971 for 2024 and 25,585 for 2025, while the national vector borne disease control programme records 9,121, 27,378 and 23,407 for those years.
  • The gap is a case definition, the sentence that decides what may be counted, and not a dispute about who fell ill. One series admits a case only on a laboratory confirmation by an NS1 or IgM test; the other also admits a rapid diagnostic test done at the point of care.
  • The two laboratory tests read different weeks of the same illness. NS1 is an antigen, a protein made by the virus itself, present in the first days of the fever. IgM is an antibody the body raises, and it appears only after several days.
  • Deaths are recorded identically in both series, 12 in 2023, 13 in 2024 and 12 in 2025, because a death is harder to define away than a case. Earlier peaks were 2012, with 13,204 cases and 66 deaths, and 2017, with 23,294 cases and 65 deaths.
  • The State has reported 17,277 dengue cases so far this year, along with 4,648 cases of scrub typhus. More than 25,000 mosquito control workers have been deployed, the ELISA test is available at every district hospital and medical college hospital, and the health helpline is 104.

Update, October 9: The State Count Reaches 21,655, and the Season Ahead

  • The Health Minister has said that 21,655 dengue cases were reported in Tamil Nadu between 1 January and 7 October, with ten deaths in the same period. 301 patients are in hospital across the State.
  • In Chennai the numbers have stabilised and hotspots have come down from 590 to 300. 3,700 workers are deployed on mosquito control in the Greater Chennai Corporation area, with each household to be visited once in seven days, and about 1,000 fever screening camps are held across the State each day.
  • On enforcement, the Corporation has collected Rs. 7.5 lakh in fines over two months from premises where breeding sources were found. The fine is Rs. 10,000 for a first violation and rises with each later violation by the same person.
  • The Minister's warning is the part to read against the calendar: he expects a fresh rise after the northeast monsoon, because standing rainwater multiplies breeding sites. The vector breeds in small collections of clean, still water in and around houses and bites during the day, so clearing sources before the season begins is the preventive step rather than treatment after it.

Update, October 18: The Field Study Gets Its Pre-Approval Letter at Kadayanallur

  • At the Tenkasi district government hospital campus at Kadayanallur, Health Minister K.G. Arunraj handed over the pre-approval letter for the field study, which makes Tamil Nadu the first State in the country to take the vaccine to the field.
  • The letter went to scientists of the National Institute of Epidemiology, the Indian Council of Medical Research institute at Chennai that studies how disease spreads in populations. Environment and Climate Change Minister V.K. Rajiv and the Director of Public Health and Preventive Medicine took part.
  • The regulatory position was restated at the event: the vaccine is cleared in more than 40 countries, carries World Health Organization prequalification, and in India is approved by the Central Drugs Standard Control Organisation for ages 4 to 60.
  • It is given in two doses three months apart and covers all four dengue serotypes, which is the point of it: recovery from one serotype gives lasting immunity to that one alone.
