Who will be covered?

Everyone in Tamil Nadu aged 70 or above can be enrolled, whatever the family income. According to the Times of India (17 August 2026), the 38 lakh people include the elderly in the 1.45 crore families already under the State insurance scheme, elderly people in families above the poverty line who hold ordinary ration cards, and people living in old-age homes who have no identity cards. Officials told The Hindu that people above 70 will be enrolled under this scheme irrespective of income.

How does it fit with CMCHIS and PM-JAY?

The Chief Minister's Comprehensive Health Insurance Scheme (CMCHIS) covers families with an annual income below Rs.1.20 lakh. It was integrated with the Union Government's Ayushman Bharat PM-JAY in 2018, and in 2024 the Union Government extended PM-JAY to all senior citizens aged 70 and above, irrespective of income. The new State scheme will be integrated with PM-JAY in the same way (The Hindu, 19 August 2026). The amount of cover has not been announced. CMCHIS then gave Rs.5 lakh per family a year, with costly procedures such as transplants paid from a special corpus. Two days later, on 19 August 2026, the Chief Minister announced that the CMCHIS cover will rise to Rs.25 lakh.

Why does Tamil Nadu need it?

The Health Department said the scheme is part of its effort to care for the State's ageing population. Figures reported with the announcement:

  • nearly 16 of every 100 residents are now aged 60 or more;
  • the share of people aged 60 and above is projected to reach 20.8% by 2036;
  • the total fertility rate has fallen from about 3.9 in 1971 to 1.3 today;
  • a Reserve Bank of India study (January 2026) projected Tamil Nadu's old-age dependency ratio at 32.7% by 2036, second among major States after Kerala.

The Health Department described the scheme as one part of its geriatric outreach, along with the 1,000 Mobile Geriatric Treatment Centres announced in the revised Budget of 5 August 2026 (Rs.33 crore), which are to take a doctor, a nurse and a physiotherapist to every village panchayat.

What else was announced on 17 August 2026?

  • Nalam 360: a free annual health check-up of 14 basic tests for everyone aged 30 and above, Rs.51 crore;
  • the Tamil Nadu Public Health Act, 1939 to be replaced by a new Act;
  • TN Medicity, an integrated medical hub, with Rs.10 crore for the project report;
  • 2,500 permanent staff nurse posts.
