Antimicrobial Resistance has emerged as one of the most pressing global health threats, affecting human health, veterinary practices, aquaculture, agriculture, waste systems, and the entire food chain simultaneously. India's newly released National Action Plan on Antimicrobial Resistance 2.0 for the period 2025-29 arrives at a critical juncture when antibiotic residues, resistant organisms, and environmental discharge connect multiple sectors in powerful ways. AMR does not remain confined to hospitals but moves through soil, water, livestock, markets, and food systems, making it a true One Health challenge. While NAP-AMR 2.0 represents a more mature and implementation-oriented framework compared to its predecessor, its fundamental success depends on addressing the critical gap in Centre-State coordination that hampered the first plan. In India's federal structure, where key determinants of AMR fall under State jurisdiction, national guidance alone cannot create uniform implementation when operational levers sit elsewhere.
1. Evolution from the First Action Plan: Progress and Limitations
The first National Action Plan on AMR launched in 2017 was a significant step forward, bringing AMR into national consciousness and encouraging multi-sectoral participation across departments.
It improved laboratory networks, expanded national surveillance systems, supported antimicrobial stewardship programmes, and placed AMR firmly within a One Health framework.
Despite this progress, implementation during the first plan period remained severely limited at the State level due to jurisdictional and administrative constraints.
Only a small group of States including Kerala, Madhya Pradesh, Delhi, Andhra Pradesh, Gujarat, Sikkim, and Punjab developed formal State Action Plans during the entire period.
Among these States, only a few moved meaningfully into actual execution, while the majority continued relying on fragmented activities within individual sectors.
State-wide multisectoral One Health structures did not take shape in most parts of the country, leaving the national framework without adequate ground-level implementation.
This slow uptake occurred not due to shortage of national effort but because health administration, hospital functioning, pharmacy regulation, and veterinary oversight are controlled by State departments.
NAP-AMR 2.0 moves beyond broad intent and outlines clearer timelines, responsibilities, and resource planning compared to its predecessor, representing mature policy thinking.
An important advancement is recognition that India cannot address AMR without meaningful private sector participation, which delivers major proportion of healthcare and veterinary services.
The plan strengthens its scientific base by emphasizing innovation including rapid diagnostics, point-of-care tools, alternatives to antibiotics, and improved environmental monitoring.
Surveillance structures are more integrated across human, veterinary, agricultural, and environmental sectors, creating a harmonized national approach to data collection.
National oversight has been elevated by placing intersectoral supervision under NITI Aayog through a dedicated Coordination and Monitoring Committee with dashboard reporting.
Despite these developments, the fundamental weakness remains unchanged as the plan stresses State participation but creates no mechanism to ensure compliance.
There exists no formal Centre-State AMR platform, no joint review mechanism, no statutory requirement for States to notify plans, and no financial pathway for sustained commitment.
India needs a national-State AMR council chaired by the Union Health Minister and guided by NITI Aayog for regular review and coordinated problem-solving across sectors.
The Union Government should formally request each State to prepare and notify its AMR Action Plan with specific timelines and mandatory annual reviews.
Experience from the National Tuberculosis Elimination Programme demonstrates that regular joint reviews, shared monitoring missions, and clearly defined roles enable effective implementation.
High-level communication through Chief Secretaries can significantly shift administrative attention, as proven by National Health Mission experience across States.
Even modest conditional grants under NHM can drive improvements in surveillance, stewardship, infection control, and laboratory strengthening when funding signals priority.
States respond with administrative energy and policy focus when financial mechanisms clearly indicate national priorities requiring State-level action.
Without structured political engagement, administrative follow-through, and shared accountability, even well-crafted national plans remain documents of intentions rather than frameworks for action.
The NAP-AMR 2.0 provides the scientific and strategic foundation India needs to combat antimicrobial resistance effectively. However, AMR is driven by real-world practices along the entire One Health continuum from hospitals and farms to markets, food chains, and wastewater systems. Without strong State participation anchored in formal coordination mechanisms, financial incentives, and accountability frameworks, national plans cannot achieve national impact. India has an opportunity now to build a coordinated Centre-State model for AMR control that could set an international example for federal health governance in addressing complex cross-sectoral challenges.
